Registration Fees
The Registration Fee for this
event is $440.00 (USD) which permits you to view each of
the conference webcast presentations once until August 31,
2008. Many discounts are available, so read
further.....
Fees for CE Credit
If you wish to receive
Continuing Education (CE) credit from one or more of the
Academy's approved
sponsors, please add a total of $45
to your registration fee.
After you have reviewed and existed a presentation, a
pop-up window will automatically appear with five
true/false questions based on the presentation content. (Be
sure you have turned off any pop-up window blockers!). The
window will also include an evaluation survey, consisting
of five questions.
To earn CE credit, you must listen to the entire
presentation and complete both the quiz and survey. If you
pass the quiz (3 or more correct of 5 T/F questions with
three chances to pass), you will then be sent an email with
your CE certificate attached as a .pdf file.

Co-Sponsor Discounts
If you have a
Co-Sponsor Discount Code that was sent to you by one of
the event's organizational co-sponsors, you can enter that
code on the Registration Form where indicated, and save an
additional 10%, reducing your registration fee to $395.
Only one co-sponsor code per participant is permitted.
Student Discount Scholarships
A limited number of reduced
tuition fee scholarships are available for full time
students enrolled in a regularly matriculated, accredited
educational institution. To apply online for a Student
Discount Scholarship, please click
here. You can also mail or FAX a
copy of your student registration card along with a
brief (1 page or less) letter explaining why you need a
scholarship and why it is important for you to attend
this event.
If you awarded a Student Discount Scholarship, you will
receive a
Scholarship Discount Code which you must enter on the
Registration Form that follows to be eligible for the
Scholarship Discount Fee of $355. You are urged to apply
early, for Student Discount Scholarships will be awarded on
a first-come, first-serve basis until the supply is
exhausted.
Special Needs Scholarships
A limited number of reduced
tuition fee scholarships are also available for those who
are disabled, handicapped, physically or psychologically
challenged, and for those with other special needs. To

here. You can also mail or FAX a
prescription or letter from your health care provider
stating the diagnosis of your special needs along with a
brief letter explaining why you need a scholarship and
why it is important for you to attend this event.
If you awarded a Special Needs Scholarship, you will

Registration Form to be eligible for the Scholarship
Discount Fee of $355. Apply early, for Special Needs

Group Registration Discounts
We are sorry that we are unable
to provide any discounts for groups of registrants who wish
to participate together from a single location. To permit
accurate student tracking, our webcast vendors require each
participant to obtain and log on and off with a unique IP,
ID and password. For those requesting CE credit, these
login records (and post-test results) will be used to
verify the presentations you’ve attended. (For more

Registration Fee Summary
Refund Policy
Your registration fees, less a
$50 administrative processing fee, will be refunded when
written notification is received by AGI on or before May 1,
2008. Fees are not refundable for any reason after May 1,
2008. Notification of cancellation must be in writing and
substitutions will be accepted with written and signed
authorization from the original registrant if received
before May 1, 2008.
